There are two "types" of visualization. One is where you see the object against some type of mental "screen". This is usually the kind referred to. You actaully see the object, and it registers with your visual sense. Some people have a difficult time with this, but can usually practice and achieve at least a useful skill at it. The other is when you imagine the object. Your not actually "seeing" it with a visual impression. This is akin to the awareness of an object (or whatever) when you remember something. This often seems to occur somewhere above your head . . for me (depending on what I'm doing visualization-wise) its above and outside my forehead hairline. Sometimes it occurs directly (or slightly infront of) the top center of my skull. This kind of mental object awareness can be initiated or assisted by considering other sense modalities (texture, smell...)
Just my opinion, the second is more difficult, more powerful, and more directly accessing the subtle or astral body. (its also the one I think most adepts are referring to when suggesting one "visualize" something in the course of magickal work)
